Revocable
Trust
  • Feel like a will is too final and afraid it can’t grow or change to fit your needs? A revocable trust is a modern planning document that is similar to a will in that you state where you want your assets to go when you pass away.
Last Will & Testament

Upon a person’s death, their assets are passed to the those legally entitled to receive them.  The court practice overseeing this is called the probate process.   If you create a will, you may indicate who receives which assets. If you do not create a will, state law will determine who receives them.
Medical
Directive

Medical Directives allow you to express your medical wishes and intentions if you are ever unable to do so, especially regarding artificial life prolonging measures if you are ever terminally ill, permanently unconscious, or in the end-stage of a fatal illness.

Healthcare Power Of Attorney

This document allows you to name someone to make medical decisions for you if you ever unable to do so for yourself. 
Durable Power Of Attorney for Finances

A Durable Power of Attorney for finances allows you to name someone as your Agent (or someone who can sign documents for you). Your Agent must act in your best interests and may handle your finances or other obligations, such as paying your bills. 
Medical Release Authorization

Medical Release Authorizations allow your agent to access medical records in specific situations that medical privacy laws may otherwise prevent. It allows your agents to access the medical information needed to act in your best interest.  
Educational Records Release Authorization

This form allows a parent, or other designated individual, to access educational information and communicate with the child’s school personnel without any privacy issues. It is also known as a Federal Education and Privacy Rights Act (“FERPA”) Waiver.
